Provision of Care

 

Prior to Admission:

  • Assessment to identify presenting Substance Use and/or Behavioral Health needs;

  • A Health Screen by a Registered Nurse to identify Neurological, Respiratory, Cardiovascular, Gastrointestinal, Musculoskeletal, Genitourinary, Infections, Skin and Wound Assessment needs

  • Review of current medication regimens and an assessment of medical stability;

  • Identification of special needs that might need to be addressed during treatment such as the need for oxygen, fall risk, seizure risk, and dietary needs;

  • Identification of known diagnoses, care providers, and existing treatment plans;

  • Identification of your rights as a patient in our care and our dedication to professionalism and confidentiality; and

  • Early identification of Discharge Planning needs

 
 
 
 

We strive to make your Treatment Process as beneficial as possible. Care is tailored to meet your individual needs. We believe that the moment you ask for help, we have a responsibility to respond. In additional to daily life skills care, individual residential rooms, and meals, below is an example of what you will receive from our staff of highly trained and caring professionals.   

Licensed by the Washington State Department of Health to provide:

  • Voluntary Acute Withdrawal Management. (ASAM, Level of Care 3.7, LOCUS Level 5)

  • Voluntary Crisis Stabilization. (LOCUS Level 5)

Day 1

Once admitted to our care, your path of care will be structured around your specific, identified needs and desired outcomes.  You may receive the following to aid in treatment delivery:

  • Medication Assessment by Licensed Prescriber

  • Urine / Blood labs draws to best help our medical staff chart a medical course of care for you;

  • We will routinely monitor your vital signs as your treatment progresses;

  • Tuberculosis Screening by a Registered Nurse;

  • Review of Oral Healthcare Habits and Needs;

  • Level of Care (LOCUS) Assessment to include life dimensions such as Risk of Harm, Functional Status, Medical, Addictive and Psychiatric Co-Morbidity, Recovery Environment, Level of Support, Treatment and Recovery History, and Engagement;

  • Assessment of abilities to perform Activities of Daily Living;

  • The development of a Health Care Plan by the Registered Nurse;

  • American Society of Addiction Medicine (ASAM) Multidimensional Assessment provided by Chemical Dependency Professional / Chemical Dependency Professional Trainee to evaluate the following life areas:

1.  Acute Intoxication and/or Withdrawal Potential

2.  Biomedical Conditions and Complications

3.  Emotional, Behavioral, or Cognitive Conditions and Complications

4.  Readiness to Change

5.  Relapse, Continued Use, or Continued Problem Potential

6.  Recovery / Living Environment

  • A thorough Mental Status Examination by a Mental Health Professional to include assessment of Affect, Mood, Thought Process, Insight and Judgement, Suicidal Ideation, Homicidal Ideation, and overall level of cognitive control;

  • The Development of an Individual Treatment Plan by a Mental Health Professional and/or a Substance Use Disorder Professional / Substance Use Disorder Professional Trainee;

  • Individual therapeutic intervention sessions to focus on identified needs;

  • Group participation to focus on a variety of topics such as problem-solving, coping, readiness to change, addiction, mental health, self-awareness, self-efficacy, goal setting and achievement and so forth;

  • COWS or CIWA Assessments as deemed medically necessary for Withdrawal Management; and

  • Discharge Planning Coordination

 

Day 2

  • Continued Medication Management from Registered Nurse Staff;

  • Continued therapeutic Individual and Group Process based on Treatment Plan;

  • COWS and CIWA re-assessments as deemed medically necessary for Withdrawal Management;

  • Industry Standardized Medical Titration and Withdrawal Management for medical detoxification; and

  • Discharge Planning and Care Coordination

 
 

Day 3 and beyond

Once again, your course of care will be crafted based on your presenting needs.  You will continue to receive the services identified above until you no longer require our level of specialty care.  Each day you will work closely with Medical, Substance Use, and Behavioral Health staff to address your needs and to actively plan for your discharge and follow-up.  Whether you are starting the process for treatment on an inpatient level or if you will be returning to your home, we are there to assist and coordinate care as your advocate and guide.  Much of our mission is based upon the reality that Recovery is a process.  This process requires ongoing support from trained professionals as well as your family, friends, and loved ones.  We will help you identify your next steps in your journey and will ensure that you leave with the best possible after-care plan in place.  We will even reach out to you by phone within three days after your discharge to see how you are progressing and to offer any additional support we can.
